| 2009 Congress of Neurological Surgeons Annual Meeting registrations will not be processed without a valid credit card or check.
Courses, seminars and event tickets will not be received without payment. |
| Registration Cancellation Policy |
| Full registration refunds, less a $100 processing fee, will be granted if written requests for cancellation are received by
September 30, 2009. Course, seminar and event tickets will be refunded in full until September 30, 2009. No refunds of any kind will be given after this date.
Refunds will not be given for no-shows. Written requests may be e-mailed to cns@jspargo.com, faxed to 703-502-0257 or mailed to 2009 CNS Annual Meeting 11208 Waples Mill Road, Suite 112, Fairfax, VA 22030. |
| Hotel Cancellation or Changes |
Prior to September 29, 2009 at 11:00 am, Eastern Time - Changes are subject to availability and cancellations may be made online at www.cns.org or via e-mail
at cns@jspargo.com.
October 1, 2009 at 11:00 am, Eastern to 72 Hours Prior to Your Scheduled Arrival Date - Please contact the assigned hotel directly with all
changes or cancellations. Hotel cancellations will receive a full refund of the $250 hotel deposit from the assigned hotel directly. The hotel will retain the $250 hotel
deposit for cancellations not made at least 72 hours prior to your confirmed arrival date. This cancellation policy is strictly enforced and no exceptions will be made.
Please retain the cancellation confirmation from the CNS Housing Center. The cancellation number provided to you by the hotel as proof of cancellation will be
required to resolve any disputes. |
